Set up your Future Super Pension Plan

It can take as little as 10 minutes to join and transfer your super into your new Pension Plan account. Important information about the Future Super Pension Plan is contained in the Product Disclosure Statement, Financial Services Guide and the Privacy Policy.

Verify your identity

Before we can create your pension account, we are required to verify your identity. For most people, we can complete this process online.

The details you've provided in the previous sections of the form will be locked when you press Continue, so if you're missing a middle name or have used your preferred name instead of your full name, please correct these before continuing.

What's your tax file number?

You can find your TFN on statements you've received from the Tax Office, your super fund, your work payment summary, or alternatively you can contact the Tax Office on 13 28 61 for help finding it. A quick search of your emails might find it too!

By providing your TFN you give Future Super permission to use your TFN for superannuation purposes. This includes creating your account and actioning any rollovers which you request.

Your retirement status can influence your superannuation and taxation options.

Funding your pension account

Which of the following methods will you use to add funds to your pension account? Please select all that apply.

Once you have commenced receiving payments from your pension account, you're unable to transfer more funds into your account.

By transferring your super to Future Super, you understand that this transfer may close your account with your other super fund.

Making a personal contribution

You can make a personal contribution of funds into your pension account when it is created. We will send you the specific payment details as part of your welcome email.

Which payment method will you use to make your personal contribution?

Making a contribution via cheque may result in delays in your first payment due to additional processing time.

How often would you like your payments?

Once your account is set up you can start receiving an income from your pension.

Payments are made via direct deposit on the 15th day of the month, or the last business day before the 15th day of the month when it falls on a weekend day or public holiday.

How much income would you like to receive per year as pension payments?

The minimum amount you can withdraw as income from your pension varies according to your age. Page 11 of the Product Disclosure Statement sets out the minimum amount of income you must receive from your pension account.

All the details I have provided are true and correct, and I have read and understood the Pension Plan Product Disclosure Statement, Financial Services Guide, Privacy Policy and other important information about joining the Future Super Pension Plan, and I am choosing to have 100% of the balance of my Future Super account invested in the Future Super Balanced Growth pension investment option.

Interests in Future Super are issued by Diversa Trustees Limited (ABN 49 006 421 638, AFSL 235153) as trustee of the Future Super Fund (ABN 45 960 194 277). The Founder and Sponsor of the Fund is Future Super Services Pty Ltd (ABN 34 619 076 023, AFS Representative No. 001255665), which is a Corporate Authorised Representative of Future Superannuation Holdings Pty Ltd (ABN 90 167 800 580, AFSL 482684). Transactions and unit prices dated 23 June 2017 and earlier relate to the member statements and investment performance history of Future Super when it formed part of the Grosvenor Pirie Master Superannuation Fund Series 2.
